Must Read:*
In consideration of being allowed to participate in the Grace Life Church of the Nazarene, Inc. sand volleyball program (dba New Albany Sand Volleyball) and related events and activities, the undersigned acknowledges, appreciates, and agrees that:
1. There is risk of injury from the activities involved in this program and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,
2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,
3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,
4. I, HEREBY RELEASE AND HOLD HARMLESS Grace Life Church of the Nazarene their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors and advertisers used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL INJURY or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
5. Allow Grace Life Church of the Nazarene, Inc./New Albany Sand Volleyball to post pictures that may include you and/or your team on its website.
I have read this release of liability and assumption of risk agreement, fully understand its terms, understand that I have given up substantial rights by signing it, and sign it freely and voluntarily without any inducement.
Pet Policy and Liability Waiver*
We welcome well-behaved pets on a leash to our premises. By bringing your pet onto our property, you agree to the following terms and conditions:

Responsibility: You are responsible for your pet at all times and must ensure they are kept on a leash and under control.

Behavior: Your pet must be well-behaved and non-disruptive to other customers and employees. Aggressive behavior will not be tolerated.

Cleanliness: You must clean up after your pet and ensure they do not cause any damage to our property.

Liability: You acknowledge that you are solely responsible for any injury or damage caused by your pet. You agree to indemnify and hold us harmless from any claims, damages, or liabilities arising from your pet's presence on our premises.

Exclusion: We reserve the right to ask you to remove your pet from our premises if they are causing a disturbance or if you fail to comply with our pet policy.
